About the Role:

The Patient Care Coordinator will provide general nursing care to patients in an outpatient clinic setting and support the health and wellness of our communities.

The ideal candidate will have excellent communication and interpersonal skills, with the ability to work effectively in a team environment. Key responsibilities include:

  • Providing comprehensive nursing care to patients
  • Collaborating with healthcare professionals to ensure seamless patient care
  • Developing and implementing individualized care plans
What We Offer:

In return for your hard work and dedication, we offer a competitive salary range of $65,000 - $85,000 per annum, depending on experience. Our benefits package includes:

Requirements:

To be successful in this role, you will need:

  • A registered nurse (RN) license or equivalent
