About the Role
We are seeking a certified care manager to join our team at our Parkway Place community in Houston, TX. As a care manager, you will play a vital role in ensuring the delivery of high-quality care to our residents while promoting their independence and dignity.

Key Responsibilities:
* Develop and implement comprehensive care plans that address the unique needs of each resident
* Coordinate with healthcare professionals to ensure seamless care transitions and effective communication
* Provide education and support to residents and their families on various aspects of care management
* Monitor and evaluate the effectiveness of care plans and make adjustments as needed

Requirements:
* Graduate of an accredited school of nursing with current licensure in Texas
* 2+ years of experience in long-term care or a related field
* Strong leadership and communication skills
* Ability to work collaboratively as part of a multidisciplinary team

Estimated Salary Range:
$65,000 - $85,000 per year, depending on experience and qualifications
