Nurse Liaison Specialist

We are seeking a skilled Nurse Liaison Specialist to join our team at Reimagine Care, an innovative company revolutionizing oncology care. As a key member of our Virtual Care Center, you will play a vital role in providing exceptional patient care and leading clinical operations.
Estimated Salary: $120,000 - $150,000 per year
Job SummaryThis is a tiered position with opportunities for growth and professional development. You will lead and optimize patient enrollment, orient and educate the care team and patients to Reimagine Care services, and provide clinical care at designated client sites.
Key Responsibilities- Develop positive working relationships with patients, providers, and care teams.
- Standardize, refine, and improve workflow processes for patient, provider, and care team engagement and satisfaction.
- Review and analyze patient enrollment, patient experience, and provider experience to identify trends, gaps, and areas for improvement.
- Initiate, engage, and collaborate with client operational and clinical teams to understand business and clinical needs aligned with established goals and metrics.
- Provide patient care and adhere to Reimagine Care policies, procedures, pathways, and communication and documentation best practices.
- Bachelor of Science in Nursing (BSN)
- Current and valid Registered Nurse license in the state of practice
- Minimum of 5 years of clinical experience in oncology
- Strong quality and patient/client-centered focused
- Ability to work within a team and as an individual contributor
- Comfortable in a fast-paced, changing environment
- Ability to work in a self-directed environment
- Excellent interpersonal skills and ability to work with a wide range of individuals
- Highly collaborative and capable of performing in a matrixed environment
- Strong problem-solving skills
