JOB SUMMARY AND QUALIFICATIONS:
The primary role of the Case Manager is to deliver case management services to families residing in Volunteers of America's housing programs. This position involves developing and overseeing a personalized service plan for each client, aimed at empowering them, enhancing their self-sufficiency, and minimizing the risk of recurring homelessness. The Case Manager will assist individuals and families in accessing essential medical, social, and educational resources, while also fostering relationships with landlords, communicating with property owners, conducting housing inspections, disseminating program information, and promoting the program within the community.
WHAT YOU SHOULD HAVE FOR THIS ROLE:
A Bachelor's degree in social services is preferred, along with 1 to 2 years of experience. Knowledge of crisis intervention and familiarity with community resources are essential. The Case Manager position requires flexibility to accommodate the schedules of residential homeless programs, strong communication skills, a valid driver's license, reliable transportation, and attendance at necessary meetings.
RESPONSIBILITIES:
1. Familiarity with community resources for homeless families.
2. Identify specific housing opportunities for families in residence.
3. Establish cooperative arrangements with government agencies involved in housing provision for homeless families.
4. Conduct regular meetings with residents to assist them in applying for and securing housing opportunities.
5. Maintain all required records and reports related to permanent housing goals for the facility and oversight agencies.
6. Keep an individual file for all clients, documenting housing history and community resource access.
7. Conduct quarterly reporting.
8. Perform quality improvement duties as assigned by supervision and the PQI committee.
9. Assist in facilitating life skills training as requested.
10. Undertake other duties as assigned.
