Job Title: Outreach Specialist

Community Connections is seeking a highly motivated and compassionate Outreach Specialist to join our Homeless Services Program. As a key member of our Outreach Team, you will play a vital role in providing referral resources and engagement services to individuals experiencing unsheltered homelessness in the District of Columbia.

Key Responsibilities:
  • Support clients in articulating their strengths, needs, and personal goals for recovery.
  • Act as an advocate for clients' integration into their communities of choice.
  • Maintain knowledge of the District's Coordinated Assessment and Housing Placement protocols.
  • Utilize community-wide outreach methods to engage with clients.
  • Provide assistance with basic life-sustaining items and accessing entitlement and other benefits.
  • Assist clients in obtaining identification and documentation for housing readiness.
  • Navigate clients through the housing application process and lease stable housing.
  • Collaborate with government partners and community providers.
  • Maintain credentialing in the SOAR model and utilize it to assist individuals in applying for Social Security disability benefits.
  • Participate in after-hours phone consultations and community-based support for emergent issues.
Requirements:
  • High School Diploma or GED with at least 3 years of relevant experience in human services.
  • Bachelor's Degree in psychology, social work, or a human service field preferred.
  • Experience working with adults experiencing mental health, substance use, and trauma preferred.
  • Valid driver's license, good driving record, automobile insurance coverage, and access to an automobile required.
  • Excellent oral and written communication skills.
  • Proficiency in Microsoft Office Suite software and electronic health record systems.
Desired Knowledge/Skills/Abilities:
  • Experience with Homeless Management Information System (HMIS) a plus.
  • Ability to read, analyze, and interpret documents.
  • Excellent organizational skills and attention to detail.
