Registered Nurse Case Coordinator

RN Case Manager
The RN Case Manager plays a pivotal role in the healthcare continuum by collaborating with various internal and external stakeholders to manage patient care from the point of admission through to transfer or discharge. This position emphasizes the efficient use of hospital resources, aiming to reduce out-of-pocket costs for patients while providing exceptional support to both internal and external clients.
Key Responsibilities
- Assess discharge requirements, determine suitable levels of care for patient transitions, and coordinate planning to resolve continuity challenges, ensuring a smooth patient care experience and minimizing readmission risks.
- Ensure that all relevant clinical information is communicated to external agencies. Effectively relay important information to the patient care team members in a professional manner. Proactively identify and address barriers to effective patient management, fostering improvements in departmental and hospital processes.
- Maintain accountability for professional growth and licensure. Commit to ongoing education and advocate for the legal, social, and ethical rights of patients while balancing their resources with the financial stewardship of the hospital. Identify quality concerns and inappropriate service utilization, referring them to the appropriate channels. Facilitate patient and family education.
Qualifications
- Bachelor's degree from an accredited nursing program.
- Current registered nurse license in the State of Ohio.
- A minimum of 3-5 years of recent clinical experience in an acute care environment.
