RN Case Manager
The RN Case Manager is responsible for collaborating with various internal and external healthcare professionals to oversee and coordinate patient care from the moment of admission through to transfer or discharge. This role emphasizes the efficient use of hospital resources, minimizing patient expenses, and ensuring high-quality customer service for both internal and external stakeholders while advocating for patient needs.
Key Responsibilities:
- Assess discharge requirements, identify suitable levels of care for patient transitions, and facilitate planning to resolve continuity challenges, ensuring a smooth continuum of care and reducing the likelihood of hospital readmissions.
- Ensure that all necessary clinical information is communicated to external agencies. Effectively relay pertinent information to the patient care team members in a professional manner. Take initiative to identify and address barriers to effective patient management and promote enhancements in departmental and hospital processes.
- Maintain accountability for professional growth and licensure requirements. Engage in self-directed learning. Advocate for the legal, social, and ethical rights of patients while balancing their resources with the financial stewardship of the hospital. Report quality concerns and inefficient service utilization to the appropriate channels. Coordinate and facilitate educational resources for patients and families.
Qualifications:
- Bachelor's degree from an accredited nursing program.
- Valid nursing license to practice in the State of Ohio.
- A minimum of 3-5 years of recent clinical experience in an acute care environment.
