RN Case Manager
The RN Case Manager plays a pivotal role in the healthcare team, collaborating with various internal and external professionals to oversee patient care from the point of admission through to transfer or discharge. This position emphasizes the efficient use of hospital resources, reducing out-of-pocket costs for patients, and providing exceptional support to both internal and external stakeholders while advocating for patients' needs.
Key Responsibilities
- Assess discharge requirements, identify suitable levels of care for patient transitions, and coordinate planning to resolve continuity challenges, ensuring a smooth continuum of care and minimizing the risk of hospital readmission.
- Ensure that all necessary clinical information is communicated to external agencies. Effectively and professionally relay pertinent information to the patient care team, taking initiative to identify and remove obstacles to efficient patient management while enhancing processes within the department and the broader healthcare community.
- Maintain accountability for professional growth and licensure. Engage in self-directed learning and advocate for the legal, social, and ethical rights of patients while balancing available resources with the financial stewardship of the hospital. Address quality concerns and inappropriate service utilization with the relevant authorities. Facilitate education for patients and their families.
Qualifications
- Bachelor's degree from an accredited nursing program.
- Valid nursing license in the State of Ohio.
- A minimum of 3-5 years of recent clinical experience in an acute care environment.
