Mobile Crisis Case Manager

Pawnee Mental Health Services is seeking a highly skilled and compassionate Mobile Crisis Case Manager to join our team. As a Mobile Crisis Case Manager, you will provide community-based mobile crisis care to individuals in need, working collaboratively with hospitals, medical and behavioral health providers, law enforcement, and other social services.
Key Responsibilities:
- Respond to clients in crisis, providing crisis interventions in a community-based setting.
- Triage and screen for suicidality, de-escalate and present resolutions, coordinate with medical/behavioral health services, and create a crisis plan and follow up.
- Follow up with clients to determine if their referred services are being provided in a timely manner and meeting their unique needs.
- Actively engage with the client in crisis planning, creating a range of tools and techniques they can use in their safety plan.
Requirements:
- Bachelor's degree in a human services field or equivalent work experience.
- A valid driver's license, a clean driving record, and automobile liability insurance.
- Excellent communication skills, both written and verbal, using the English language.
- Basic computer skills.
Preferred Qualifications:
- Knowledge of the needs and challenges of adults who experience severe and persistent mental illness.
- Prior experience in a mental health crisis setting, or working with individuals experiencing a mental health crisis.
- Experience with Microsoft Office.
