Job Summary

We are seeking a highly skilled Registered Psychiatric Nurse to join our team as a Community Outreach Specialist. In this role, you will provide screening assessments and psychiatric nursing services to youth and young adults struggling with severe mental illness and co-occurring substance use.

Key Responsibilities
  • Conduct comprehensive assessments to identify client needs and develop individualized treatment plans
  • Provide evidence-based interventions and support services to promote mental health and wellness
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and referrals
  • Develop and maintain relationships with community providers and stakeholders to enhance service delivery
  • Participate in quality improvement initiatives to ensure best practices and outcomes
Requirements
  • Associate or bachelor's degree in Nursing
  • Minimum of two years experience working as a psychiatric nurse in a community-based behavioral health setting
  • Valid and current Rhode Island Registered Nurse License
  • Valid driver's license and reliable transportation
  • Familiarity with screening/assessment tools, local care coordination resources, and evidence-based practices
  • Oral and written communication skills at a proficient level ensuring effective and professional communication
  • Ability to provide crisis intervention, brief counseling, or case management when determined necessary
