Job Summary

The Integration Care Facilitator plays a vital role in the Program Team, working closely with consumers to provide comprehensive support services. This position requires a strong understanding of behavioral health principles and a passion for working with diverse populations.

Key Responsibilities
  • Conduct outreach and engagement activities to connect with the target population.
  • Perform intake interviews and assessments to determine program eligibility.
  • Develop and implement individualized service plans (ISPs) to meet consumer needs.
  • Provide education and support to consumers on various topics, including employment readiness and crisis intervention.
  • Collaborate with other providers and entities to ensure seamless service delivery.
  • Maintain accurate and timely documentation of consumer interactions and program activities.
Requirements
  • Bachelor's degree in behavioral sciences or a related field.
  • Minimum of two years of experience working with homeless, incarcerated, or substance-abuse populations.
  • Strong knowledge of electronic health records and data collection systems.
  • Valid driver's license for business-related travel.

Physical Requirements
