Key Responsibilities- Coordinate care according to resident needs, monitor care provided, and oversee medication management program.
- Communicate with physicians, medical professionals, and responsible parties regarding treatments and needs.
- Ensure timely ordering of medications and audit medication carts and narcotics frequently.
- Assist with admissions, including admission evaluations and completing evaluations and assisting directors with service plans as needed.
- Respond quickly to emergencies and maintain close communication with all departments and staff members.
- Monitor documentation of Medication Assistants and Resident Assistants to ensure accuracy, timeliness, and completeness.
- Assist residents with activities of daily living, including bathing, dressing, grooming, toileting, positioning, transfer, mobility, and incontinence care, as well as administering medications when needed.
- Verify service plans are current and accurate as needed and notify supervisor of discrepancies.
- Assist with data collection for incident reports and input reports as they occur.
- Manage and facilitate orientation process for new employees on shift.
- Ensure effective communication between direct care staff and client/resident families.
- Assist with staff scheduling and attend all in-services as required.
- Maintain a professional appearance and good personal hygiene per company policies.
- Desire to work with seniors and tolerance for behaviors associated with dementia and other aging processes.
- Ability to abide by and uphold company core values.
