Position Overview

Hope for Youth is seeking a dedicated MDFT Counselor to deliver Prevention Services within the Nassau County area. As a nonprofit organization, we have a long-standing commitment to child welfare, aiming to foster positive outcomes for children, adolescents, and their families.

Role Responsibilities

The MDFT Counselor will engage in a range of activities designed to support families through:

  • Preventing disruptions that may lead to a child's placement in foster care.
  • Facilitating the timely reunification of children with their families after foster care placement.
  • Minimizing the chances of children re-entering foster care after discharge.

Key tasks include:

  • Creating a detailed clinical treatment plan that encompasses counseling, mental health, and substance abuse services tailored for youth exhibiting high levels of behavioral and trauma-related challenges, as identified by the CANS-NY.
  • Providing individual therapeutic services to a caseload of up to 12 clients, with the program accommodating a total of 50 clinical cases annually.
  • Implementing the MDFT program over a 3- to 6-month period, with sessions ranging from 30 to 60, and meeting clients 1 to 4 times weekly as required.
  • Delivering interventions in various settings, including home, community, or office environments.
  • Conducting collateral contacts in educational or community settings as appropriate.
  • Documenting all interactions through psychosocial assessments, treatment plans, progress notes, and other necessary case-related documentation.
Qualifications

To be considered for the MDFT Counselor position, candidates must possess:

  • A Master's Degree and a relevant license (LMSW, LCSW, LMHC, LMFT).
  • Proficiency in both Spanish and English is preferred.
  • Prior experience is advantageous; however, training in the MDFT model will be provided.

  • Strong written, verbal, and computer skills.
  • A flexible schedule to meet program needs.
  • Active participation in treatment teams, case reviews, and other collaborative meetings.
  • The ability to work effectively with referral resources and other agencies involved with families.
  • Experience working within interdisciplinary teams.
  • Willingness to provide support across various projects and locations, including travel as necessary.
  • Knowledge of child-serving systems and experience with adolescents.
