Job Description

At Tarzana Treatment Centers, we are seeking a highly skilled Case Manager I to join our multidisciplinary team for the CalAIM Enhanced Care Management program. This program offers in-person case management and support services to Medi-Cal members with multiple chronic conditions.

Key Responsibilities:
  • Serve as the main point of contact and advocate for Enhanced Care Management for Justice Involved (JI) enrolled members.
  • Connect enrolled members to community resources, facilitate medical referrals, set goals, address barriers, link to health and support services, establish benefits, provide transportation, manage crises, and conduct outreach within the community and TTC sites.
  • Maintain active engagement and interactions with program-enrolled members to ensure proper coordination of care.

Requirements:
  • A minimum of 1 year experience in case management.
  • A background in working with individuals who have mental health conditions and substance use disorders.
  • A flexible schedule that includes late evenings and weekends.
  • Experience with in-home visits, homeless encampments, and correctional facilities.
  • The ideal candidate should have either personal experience with the justice system or a proven history of working with individuals who are involved with the justice system.

Work Environment:

Categories of Duties:
  1. Conducts self in a professional manner in areas of dress, language, attitude, and behavior.
  2. Exhibits full knowledge and support of the philosophy and objectives of the Agency.
  3. Fully understands and maintains policies regarding professional ethics, including informing patients about other services offered by Tarzana Treatment Centers, Inc. (TTC) that may be appropriate for the patient's family members.
  4. Attends in-services trainings and educational workshops.
  5. Attends all required staff meetings.
  6. Cooperates and communicates fully with other staff members regarding patient behavior, progress activities, etc.
  7. As appropriate, works closely with other multi-disciplinary team members to develop a comprehensive Individualized Treatment Plan on assigned patients and to ensure that all identified problems are addressed.
  8. Assist with employment/vocational referrals, provide vocational counseling regarding resume preparation, job interviewing, etc.
  9. Provides outreach and follow-up services to patients in and out of treatment, to check progress, offer additional services, help re-engage in treatment, make needed referrals.
  10. Facilitates discharge-planning efforts, commencing at time of admission on all assigned patients, including areas of housing, employment, medical, psychiatric, financial, aftercare, etc.
  11. Makes direct contact within agency and with outside entities as appropriate, in order to assist patients in resolution of legal, financial, housing, medical, psychiatric, and discharge planning issues.
  12. Provides regular, comprehensive progress note charting in patient's Medical Record which document contacts, progress towards identified goals, program participation, referrals, discharge planning, and other case management activities.
  13. Performs other duties as assigned by Director and/or Administrator.
