Case Manager I

Job Summary:
Tarzana Treatment Centers, Inc. is seeking a highly skilled Case Manager I to join our multidisciplinary team. As a Case Manager I, you will provide enhanced case management services to eligible Medi-Cal members with multiple chronic conditions.
Key Responsibilities:
- Provide case management services to patients with multiple chronic conditions.
- Identify and link eligible participants to project services.
- Participate in outreach efforts at community sites and in-reach at TTC service site.
- Provide follow-up as needed to engage potential participants or those who have fallen out of care.
- Collaborate with multidisciplinary team members to develop comprehensive individualized treatment plans.
- Assist with employment/vocational referrals and provide vocational counseling.
- Facilitate discharge-planning efforts, including housing, employment, medical, psychiatric, financial, and aftercare services.
- Provide regular, comprehensive progress note charting in patient's medical record.
Requirements:
- BA/BS/BSW degree in social work or related fields required or 1 year of case management experience.
- Prior case management experience preferred, as well as experience working with individuals with mental illness and substance abuse disorders.
- Bi-lingual Spanish/English preferred.
