Position Summary:

The AIM Palliative RN will report to the Director of Palliative Care, engaging with patients, families, and healthcare professionals to deliver and coordinate exceptional end-of-life care. The AIM Palliative RN conducts both in-person and telephonic assessments for palliative care patients, working in tandem with an interdisciplinary team. The primary responsibility involves performing in-home assessments, as well as in assisted living and skilled nursing facilities, while providing education and support to patients and their families to ensure comfort in their home settings.

Key Responsibilities:

  • Conduct comprehensive in-person and telephonic assessments, providing education to palliative care patients and their families.
  • Complete Risk Stratification Assessments to determine patient support needs in collaboration with the Nurse Practitioner.
  • Monitor patient visit frequency based on individual needs and risk factors.
  • Deliver consistent, high-quality education and support to patients and families, fostering a calm and comforting environment.
  • Consult with patients and families regarding disease processes, self-care strategies, end-of-life care, and nutritional needs.
  • Communicate findings to the Primary Care Provider or AIM Palliative Nurse Practitioner regarding necessary skilled nursing or therapy interventions.
  • Educate patients and families on medication administration and treatment plans as prescribed.
  • Coordinate care with members of the palliative care team.
  • Document clinical findings and maintain accurate patient records, including assessments, physician orders, and care plans.
  • Support the development and implementation of individualized palliative care plans from assessment through discharge.
  • Participate in evening and weekend on-call services as required.
  • Facilitate prescription management and communication of clinical findings to the Nurse Practitioner and/or Primary Care Provider.
  • Establish and maintain effective working relationships with all team members and stakeholders.

Qualifications:

Education:

  • Registered Nurse (RN)

Experience:

  • A minimum of 2 years of experience as a skilled Registered Nurse.
  • Proven ability to deliver high-quality patient care.
  • Preferred experience in palliative care, oncology, hospice, or geriatric nursing.

Licenses and Certifications:

  • Current RN license to practice.
  • CPR Certification.

Skills:

  • Proficient in Microsoft Office Suite and electronic medical record systems.
  • Strong communication and problem-solving skills.
  • Ability to maintain patient confidentiality and demonstrate compassionate care.
