We are seeking a highly skilled Registered Nurse Case Manager to join our team at Interim HealthCare Home Care. As a key member of our home health team, you will play a vital role in providing exceptional patient care and support to our clients.
Key Responsibilities:
- Conduct comprehensive patient assessments and develop personalized care plans to meet the unique needs of each individual.
- Oversee the implementation and ongoing evaluation of patient care plans, ensuring seamless coordination with healthcare providers and caregivers.
- Communicate effectively with patients, families, and healthcare teams to ensure continuity of care and address any concerns or issues that may arise.
- Provide education and training to patients and families on proper home healthcare procedures, including wound care, IV administration, and medication management.
- Work collaboratively with our interdisciplinary team to identify and address potential health risks, promoting preventative care and reducing readmissions.
Requirements:
- Current unrestricted license to practice as a Registered Nurse (RN) in the state associated with this position.
- Current CPR/AED/BLS/First Aid certification.
- Reliable transportation to/from care sites and/or work locations.
- One (1) year of professional experience practicing as a Registered Nurse (RN) in home health or a similar setting; previous case management/utilization review experience is preferred.
