Position Overview:
As a vital member of our organization, the Care Coordinator plays a crucial role in ensuring the well-being of minor clients. This position involves evaluating whether a minor client qualifies for essential services and facilitating comprehensive care coordination meetings.
Key Responsibilities:
The Care Coordinator will be responsible for:
- Participating in an on-call rotation for the Care Coordination Hotline, which may include evenings and weekends, while accurately documenting necessary information.
- Maintaining detailed records for grant metrics in the appropriate database within specified timeframes, including demographic data and service provision.
- Assessing eligibility for coordinated services based on established protocols.
- Coordinating responses from advocates, law enforcement, medical personnel, and mental health providers.
- Organizing Rapid Response Team meetings within a specified timeframe following the identification or recovery of a minor.
- Collaborating with law enforcement and Child Protective Services to initiate joint investigations.
- Representing clients in multidisciplinary team meetings as required.
- Preparing all necessary materials for Care Coordination meetings, including agendas and notifications.
- Implementing safety protocols in emergency situations.
- Participating in internal staffing, supervision, and community meetings.
- Creating and maintaining an information tracking system to support reporting and volunteer management.
- Developing expertise in community resources to assist clients effectively.
- Upholding confidentiality and professional standards in all interactions.
- Traveling across regions as necessary.
Qualifications:
Candidates must meet the following requirements:
- Pass a pre-employment drug screening and maintain emotional control and professional demeanor.
- Provide proof of work eligibility upon request.
- Pass pre-employment and biennial criminal background checks.
Skills and Competencies:
The ideal candidate will demonstrate:
- Ability to respond sensitively to diverse cultural and socio-economic backgrounds.
- Advanced training in crisis management.
- Effective communication skills in both written and verbal formats; bilingual proficiency in Spanish is highly desirable.
