Responsibilities
This role involves:
  • Performing patient assessments and collaborating with the care team to develop and implement a plan of care.
  • Making referrals to other hospice care disciplines as needed or documenting the rationale for not doing so.
  • Teaching patients, families, and caregivers appropriate patient care techniques.
  • Regularly communicating with clinical managers and care teams.
  • Performing on-call responsibilities as assigned.
