We're looking for a skilled and compassionate nurse to join our team at Phoenix Home Care & Hospice. As a pediatric private duty LPN, you'll provide high-quality care to children with complex needs and work closely with our interdisciplinary team to ensure seamless care transitions.

Job Details

  • This is an overnight position, Monday through Friday, with a variety of case types, including children with g-tube, trach, and ventilator requirements.
  • The ideal candidate will have excellent problem-solving skills, strong communication abilities, and a passion for delivering exceptional patient care.

Responsibilities:

  • Develop, review, and revise plans of care in collaboration with the healthcare team.
  • Provide nursing care according to physician orders, individual plans of care, and payer source authorization.
  • Maintain accurate records of patient progress and communicate effectively with healthcare professionals.
  • Supervise and instruct clients, family members, or other healthcare team members on plan of treatment.
  • Document all care services provided using electronic charting systems.
  • Participate in continuing education programs, quality improvement initiatives, and staff meetings.

Requirements

  • A valid LPN or RN license.
  • Strong problem-solving and multitasking skills.
  • Effective communication and interpersonal skills.
  • A positive, professional attitude.
