Personal Care Specialist

Responsibilities:
- Provide one-on-one care to patients in their homes, including assistance with daily activities and personal care
- Develop and implement individualized care plans that meet the unique needs of each patient
- Communicate effectively with patients, family members, and healthcare professionals to ensure seamless care coordination
- Maintain accurate and detailed records of patient care and progress
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments
Requirements:
- Graduated from an accredited Home Health Aide program
- High School Diploma or GED
- One-year prior professional experience
- Drivers license required
- CPR certification preferred
