As a Palliative Care Nurse Practitioner at Affinity Hospice, you will play a vital role in overseeing patient care and delivering high-quality services. Your primary responsibility will be to initiate palliative care consultations or develop appropriate hospice treatment plans for patients in various settings.

Key Responsibilities:

  • Patient Assessments: Conduct comprehensive assessments of patients to determine their physical, emotional, and social needs.
  • Treatment Planning: Develop individualized treatment plans that address the unique needs of each patient.
  • Collaboration: Work closely with medical colleagues to ensure seamless coordination of care and timely documentation in Palliative Care EMR.
  • Education and Counseling: Provide patients and their families with essential information regarding their conditions, treatment options, and available resources.
  • Evaluation and Reassessment: Regularly evaluate the effectiveness of treatment plans and make necessary adjustments to ensure optimal patient outcomes.

**Benefits and Requirements**

Affinity Hospice offers a competitive salary range of $85,000 - $110,000 annually, depending on experience and location. Our benefits package includes medical, dental, vision, 401(k), short-term and long-term disability, employee assistance program, cell reimbursement, and more.

To qualify for this role, you must hold a current unrestricted license to practice as a registered professional nurse in the state of employment, a graduate degree from an approved Nurse Practitioner program, and current unrestricted licensure to practice as a Nurse Practitioner by the Board of Nursing in the state of employment. Additional requirements include current and continued CPR certification, a minimum of 2 years of palliative care and/or hospice experience, and certifications such as CHPN (Certified Hospice and Palliative Care Nurse).]
