Speech-Language Pathologist

We are seeking a highly skilled Speech-Language Pathologist to join our team at Prime Care Coordination. As a Speech-Language Pathologist, you will be responsible for providing direct and indirect speech therapy services to participants in our Article 16 clinics.
Key Responsibilities- Direct Clinical Services:
- Provide physician-ordered speech therapy services to participants.
- Ensure physician orders are renewed in a timely manner, as per OPWDD Article 16 requirements.
- Complete all clinical documentation, including initial assessments, daily notes, renewal orders, progress reports, and discharge summaries.
- Identify patient short-term and long-term goals, reflecting progress or lack of goal achievement in all charting, and confer with physicians and/or interdisciplinary teams regarding patient response to therapy.
- Complete communication and swallowing evaluations and develop appropriate communication and feeding programs.
- Help patients work towards accomplishing their treatment goals by utilizing therapeutic and assistive devices.
- Attend scheduled interdisciplinary staff meetings.
- Supervise speech therapy students and new graduates as assigned.
- Indirect Clinical Services:
- Complete observations of diet consistency, as requested, and recommend equipment and assistive devices as appropriate for dining needs.
- Update and develop Individualized Plans of Care or Conditions based on individual needs.
- As requested, evaluate and instruct patients and caregivers in safe dining practices and positioning, or make referrals to receive direct therapy through the Article 16 clinics.
- Provide training and supervision to direct care staff regarding implementation of treatment plans developed by the Speech-Language Pathologist.
- Provide consultation to other team members as needed for Speech-Language Pathologist needs.
- Instruct, enhance, and develop staff training programs for employees.
- Provide staff training and in-servicing as needed or requested, which may include choking prevention training and condition-based staff training.
- Attend scheduled interdisciplinary staff meetings, administrative meetings, case reviews, and ITTs when requested.
- Complete all assigned trainings.
- Maintain professional knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies.
- Participate in committees and program development as indicated.
- Assist the Associate Director/Manager with budgeting for Speech-Language Pathologist Department needs, requesting needed supplies and equipment.
- Perform all other duties relevant to the position as assigned by supervisor.
- Possess or be eligible for a Certificate of Clinical Competence (CCC).
- Be licensed and currently registered as a Speech-Language Pathologist by the New York State Department of Education, or eligible for licensure by virtue of holding a Certificate of Clinical Competence (CCC) issued by the American Speech-Language-Hearing Association (ASHA).
- Be at least 18 years of age.
- Have reliable transportation and a valid New York State driver's license.
- Be able to work independently and motivate others.
- Be able to communicate effectively, both orally and in writing.
- Be able to lift at least 50 pounds of dead weight.
- Maintain all required certifications and training by State regulations and CDS Life Transitions policy.
- Experience working with individuals with developmental disabilities is preferred.
