We are seeking a compassionate and skilled Certified Home Health Aide to join our team at Physicians Choice Home Health. As a Certified Home Health Aide, you will provide high-quality home health visits to our patients in the comfort of their own homes.
Responsibilities:- Provide hands-on personal care, including bathing, back rubs, oral hygiene, shampoos, and changing bed linen.
- Assist patients with dressing and undressing, skin care to prevent breakdown, and toileting activities.
- Keep patient living areas clean and orderly, plan and prepare nutritious meals, and assist with feeding if necessary.
- Take and record oral, rectal, and axillary temperatures, pulse, respiration, and blood pressure when ordered.
- Assist with ambulation and exercise according to the plan of care, and perform range of motion and other simple procedures as an extension of therapy services.
- Report on patient condition and significant changes to the assigned nurse, and adhere to Physicians Choice Home Health documentation and care procedures.
- Meets training requirements and successfully completed a training and competency evaluation program.
- Shows proof of registration on the Home Care Aide Registry, preferred.
- At least 18 years of age, ability to read and follow written instructions, and document care given.
- Self-directing with the ability to work with little direct supervision, empathy for the needs of the ill, injured, frail, and impaired.
- Possess and maintain current CPR certification, discretion in dealing with sensitive and confidential information, and ability to travel to different patients' homes.
