As a Hospice Referral Coordinator, you will play a vital role in ensuring seamless transitions from hospital to community settings, providing comfort and symptom management to patients and their families.

Key Responsibilities:
  • Act as a liaison between internal and external referral sources, ensuring timely and effective communication.
  • Process inpatient referrals into hospice, ensuring continuity of care and coordination of discharge planning.
  • Assess patient needs, develop and implement individualized care plans, and provide direct patient care in accordance with agency policies and procedures.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and support for patients and their families.
Requirements:
  • Graduate of an accredited school of professional nursing (BSN preferred).
  • Current licensure as a Registered Nurse (RN) in the State of Ohio.
  • Current Basic Life Support (BLS) certification through the American Heart Association (AHA).
  • Valid Ohio Driver's License and proof of automobile insurance coverage.
Competencies:
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment with multiple priorities.
  • Knowledge of JCAHO and Medicare standards and regulations.
Work Experience:
  • Minimum one year current clinical experience as a registered nurse.
  • Minimum one year experience in an acute medical-surgical or critical care setting.
