Position Overview

The Patient Care Program Manager is tasked with coordinating patient care initiatives and facilitating communication among all stakeholders involved in a patient's treatment journey. This role aims to enhance safety and effectiveness in treatment outcomes. Additionally, the manager will supervise support personnel, including Care Coordinators and Resource Coordinators, while embodying the core values of Resources For Human Development (RHD) in all job functions.

Key Responsibilities

Program Development and Oversight

  • Stay informed about RHD programs and ensure their effective implementation.
  • Understand all program-related regulations from funding bodies, community support services, and government licensing agencies.
  • Foster and strengthen relationships with RHD program staff and external providers to ensure coordinated care.
  • Maintain a tracking system to identify the need for comprehensive treatment plans.
  • Engage with both internal and external partner providers through regular off-site visits.

Staff Development and Oversight

  • Provide direct supervision to Care Coordinators and Resource Coordinators.
  • Lead routine collaborative planning meetings with the treatment team.

Clinical Support

  • Deliver statistics and outcome measures related to quality of care.
  • Respond to weekly and monthly reporting requirements.
  • Conduct systematic reviews to ensure thorough analysis of each client's care and confirm that charts are linked with all available resources.
  • Develop comprehensive treatment plans for clients accessing multiple services across various settings.
  • Review compliance with individual treatment plans.

Qualifications

  • Master's Degree in a Human Services field.
  • Clinical Mental Health license (LPC, LCSW).
  • Two years of experience working with children and/or adults with Mental Health Diagnosis/IDD/Drug and Alcohol Treatment.
  • Valid Driver's License.

Working Conditions

  • Required to drive throughout the program service area.

Physical Requirements

  • Sedentary: exerting up to 10 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or move objects (including self). Most work involves sitting for the majority of the time.
  • Light: exerting up to 20 pounds of force frequently, and/or negligible amount of force constantly to move objects. The use of arm and/or leg control requires force greater than sedentary, but the worker still sits for the majority of the time.
