Job Title: Hybrid Peer Specialist

Our client, The CKHobbie Group, is seeking a highly motivated and compassionate Hybrid Peer Specialist to join their team. As a Hybrid Peer Specialist, you will play a vital role in supporting individuals with Substance Use Disorder (SUD) and other mental health challenges.

Key Responsibilities:
  • Engage with members in the community and via phone, providing emotional support, encouragement, and positive reinforcement.
  • Conduct comprehensive assessments to identify clinical, psychosocial, and behavioral health needs.
  • Utilize motivational interviewing and active listening skills to help members set goals and develop personalized care plans.
  • Assist members in accessing physical and behavioral health services, including scheduling appointments and filling prescriptions.
  • Connect members with resources addressing social determinants of health and help resolve barriers to care.
  • Collaborate with healthcare providers and Managed Care Organizations (MCOs) to present complex cases and obtain input for member care plans.
  • Document interactions, assessments, and referrals in relevant systems and stay updated on technology changes.
  • Follow up with members to ensure they understand their medication and care plans.
  • Meet performance goals within community and virtual environments.
Requirements:
  • Peer Specialist Certification or lived experience with SUD/OUD/Mental Illness.
  • High school diploma or equivalent.
  • Strong passion for supporting individuals with complex medical, behavioral, or social needs.
  • Excellent communication and organizational skills, with the ability to multitask in a fast-paced environment.
  • Proficiency in Microsoft Office applications, with the ability to learn clinical software quickly.
  • Access to a designated workspace with secure, high-speed internet.
  • Valid driver's license and auto insurance, with the ability to travel for face-to-face visits and navigate Philadelphia's public transportation system.

The ideal candidate will possess a strong understanding of OUD/SUD member management and familiarity with community or clinical settings. They will also have excellent communication, interpersonal, and motivational skills to effectively engage with members.
