About the Role

We are seeking a skilled and compassionate Licensed Practical Nurse to join our team as a Nurse Coordinator for special needs patient care. This is a unique opportunity to expand your skills set, work with a special patient population, and be part of a collaborative practice team.

Key Responsibilities
  • Prepare daily assignments for nursing staff and collaborate with the Director of Nursing to establish systems for day-to-day medical and nursing operations.
  • Participate in daily huddles and practice team meetings, ensuring staff compliance with all aspects of the PCMH initiatives.
