Job Title: Nurse Care Coordinator

We are seeking a compassionate and skilled Licensed Practical Nurse to join our team at Services for the UnderServed, Inc. in Brooklyn. This exciting opportunity is located at 1000 Blake Avenue, Brooklyn, NY 11208.

The successful candidate will work closely with our Program Director, Director of Social Services, Registered Nurse, and Social Service staff to coordinate high-quality healthcare services for our clients.

Job Summary:
We are seeking an LPN who is passionate about delivering holistic care and making a difference in the lives of others. As a key member of our healthcare team, you will be responsible for:
• Providing medical care and guidance to clients
• Collaborating with multidisciplinary teams to develop and implement care plans
• Conducting regular health assessments and screenings
• Educating clients on healthy habits and disease prevention

Requirements:
To be successful in this role, you will need:
1. A valid LPN license in New York State
2. Strong communication and interpersonal skills
3. Ability to work effectively in a fast-paced environment
4. Compassion and empathy for clients from diverse backgrounds
