Position Title: Clinical Care Coordinator – LVN

POSITION SUMMARY

The Clinical Care Coordinator – LVN is an integral member of the Montage Medical Group's Collaborative Care Services (CCS) team, dedicated to providing essential support to patients and their healthcare teams. This role is pivotal in ensuring that patients who have recently been discharged from the hospital receive timely and appropriate follow-up care. The Clinical Care Coordinator collaborates closely with various healthcare professionals to address patient needs efficiently and effectively.

KEY RESPONSIBILITIES

  • Manage transitional care services to enhance patient outcomes.
  • Collaborate with the healthcare team to minimize avoidable hospital readmissions.
  • Coordinate comprehensive patient care and assess individual needs.
  • Provide telephonic guidance, manage inquiries, and engage physicians or care managers as required.
  • Facilitate seamless patient transitions throughout the healthcare continuum.
  • Evaluate patient progress towards health objectives.
  • Identify and address obstacles to adherence to treatment plans.
  • Engage in quality improvement projects to enhance service delivery.
  • Contribute to strategies aimed at managing specific patient populations.
  • Perform additional duties as assigned.

EDUCATIONAL REQUIREMENTS: Must possess a valid LVN license in California, along with a High School diploma and American Heart Association BLS certification.

PROFESSIONAL EXPERIENCE: Previous experience as an LVN in care management or related healthcare environments is essential, along with familiarity with electronic medical records systems.

ESSENTIAL REQUIREMENTS: A current California LVN license is mandatory.
