Job Overview:

The Lead Registered Nurse is accountable for the provision of patient care through the nursing process, which includes assessment, planning, implementation, and evaluation. This role involves directing and mentoring patient education initiatives and overseeing the activities of support staff while upholding the standards of professional nursing practice. The incumbent must remain informed about all relevant federal, state, and organizational regulations, policies, and standards.

Qualifications:

Entry-level position for graduates meeting educational requirements. Experience in Specialty Units preferred, with a minimum of 1 year in medical-surgical nursing (e.g., ICU, Maternity, Psychiatry).

Competencies:

Refer to core competencies and unit-specific competencies.

Licensure and Certifications:

Valid New York State license as a registered professional nurse. Current Basic Life Support (BLS) certification required (or to be obtained within the first month of employment). For Specialty Unit - Maternity: Neonatal Resuscitation Certification to be completed within 6 months of employment. For Specialty Unit - ICU: Advanced Cardiovascular Life Support (ACLS) certification to be obtained within 6 months of hire.

Key Responsibilities:

Employees may be required to perform tasks beyond those specifically outlined.

Patient Assessment: Conduct thorough patient assessments and document findings in a timely manner according to nursing and departmental policies.

Complete nursing admission assessments accurately and within designated timeframes. Conduct skin risk assessments and reassessments as per policy. Perform pain assessments and document findings using the approved pain scale.

Patient Monitoring: Monitor patients according to their condition, status, and safety protocols.

Regularly check and document vital signs and clinical status. Identify and report any changes in patient condition to the charge nurse or physician. Ensure patient safety by adhering to all policies related to National Patient Safety Goals. Raise concerns regarding patient care and utilize the Chain of Command as necessary.

Medication Management: Administer medications, IV fluids, and blood products safely and document accordingly.

Adhere to medication administration policies using electronic medication administration records (e-Mar) and maintain a minimum scan rate of 85%. Insert IVs following aseptic techniques and document appropriately. Ensure compliance with hospital policies during blood product administration. Maintain security of electronic signatures and account for all narcotics appropriately.

Patient and Family Education:

Assess and address the educational needs of patients and their families regarding their health conditions and treatment plans. Document educational needs and provide relevant teaching materials. Evaluate the understanding of patients and families post-education.

Discharge Planning:

Initiate and contribute to discharge planning upon admission and engage in interdisciplinary collaboration. Complete all pre-discharge checklist items. Provide patients with accurate discharge instructions using two patient identifiers. Ensure safe discharge and provide necessary follow-up information and community resources.

Support for Providers: Assist healthcare providers in performing procedures and treatments as per unit and hospital policies.

Support physicians with bedside procedures and maintain competency in unit-specific tasks. Utilize SBAR communication when reporting changes in patient conditions.

Equipment Proficiency:

Demonstrate knowledge and safe usage of medical equipment. Follow hospital protocols for equipment usage and patient safety.

Nursing Skills: Exhibit the ability to perform nursing treatments and procedures competently.

Conduct hourly rounds on patients and document findings appropriately. Utilize patient room whiteboards for effective communication with patients and families.

Emergency Preparedness: Demonstrate understanding of emergency preparedness protocols.

Actively participate in emergency preparedness drills and procedures.

Documentation: Accurately document patient care activities in accordance with departmental policies.

Maintain real-time documentation of all patient care activities.

Professional Development: Engage in professional nursing promotion through participation in quality improvement initiatives, community involvement, and continuing education.

Attend team meetings and participate in clinical updates and mandatory training.

Infection Control: Provide patient care in alignment with infection control policies.

Adhere to hand hygiene protocols and isolation procedures.

Utilize Nursing Process: Identify patient needs through assessment and implement appropriate nursing interventions.

Develop individualized patient care plans for all admissions. Regularly update care plans as needed.

Other Responsibilities:

Compensation: $37.50 to $53.06
