Certified Addiction Counselor Job Overview

Introduction

Unity Health Care is seeking a committed Certified Addiction Counselor (CAC) to join our dedicated team. This role involves providing specialized addiction services to individuals within correctional facilities. You will collaborate with a diverse team to assist clients facing challenges related to addiction, mental health issues, and social instability.

Key Responsibilities

  • Manage cases for patients receiving Medication Assisted Treatment (MAT) in conjunction with available resources.
  • Deliver personalized support in areas such as relapse prevention, housing assistance, and medication management.
  • Work alongside the Substance Use Disorder (SUD) team to create individualized treatment plans and recovery pathways.
  • Provide integrated services for clients with both mental health and substance use disorders.
  • Facilitate rehabilitation groups and educational workshops.
  • Maintain accurate and timely documentation in electronic health records.
  • Coordinate discharge planning and referrals to community support services.
  • Engage in team meetings, supervision, and training initiatives.
  • Perform additional duties as assigned by the SUD Program Director.

Qualifications

  • Master's Degree in Social Work or related field.
  • Licensed Professional Counselor or Certified Addiction Counselor I or II certification.
  • A minimum of three years of experience in the field of Substance Use and Co-Occurring Disorders.
  • Current CPR/First-Aid certification.
  • Proficiency in Spanish is preferred.

  • Knowledge of Medication Assisted Treatment (MAT) and harm reduction techniques.

  • Familiarity with Microsoft Office Suite and Electronic Medical Record (EMR) systems.

Supervision

This position will report directly to the SUD Program Director.

Compliance

Adhere to all relevant licensing and regulatory standards as well as organizational policies.
