Introduction

At Unity Health Care, we are seeking a committed Certified Addiction Counselor (CAC) to join our team. This role involves delivering addiction-focused support to individuals facing challenges related to substance use within various facilities. You will collaborate with a diverse team to assist clients dealing with addiction, as well as associated medical and psychological issues.

Key Responsibilities

  • Manage cases for Medication Assisted Treatment (MAT) patients in partnership with available resources.
  • Provide personalized services in areas such as relapse prevention, housing assistance, and medication management.
  • Work with the Substance Use Disorder (SUD) team to create individualized treatment plans and recovery pathways.
  • Deliver integrated services for clients with both mental health and substance use disorders.
  • Facilitate rehabilitation groups and educational workshops.
  • Maintain accurate and timely documentation in electronic health records.
  • Oversee discharge planning and connect clients with community resources.
  • Engage in team meetings, supervision, and training initiatives.
  • Perform additional tasks as assigned by the SUD Program Director.

Qualifications

  • Master's Degree in Social Work or a related field.
  • Licensed Professional Counselor or Certified Addiction Counselor I or II credentials.
  • A minimum of three years of experience in working with Substance Use and Co-Occurring Disorders.
  • Current CPR/First-Aid certification.
  • Proficiency in Spanish is preferred.

  • Exceptional communication and interpersonal skills.
  • Knowledge of Medication Assisted Treatment (MAT) and harm reduction methodologies.
  • Ability to apply critical thinking, problem-solving, and effective time management.
  • Commitment to patient confidentiality and respect for diverse recovery journeys.
  • Familiarity with Microsoft Office Suite and Electronic Medical Record (EMR) systems.

Supervision

This position will report directly to the SUD Program Director.

Compliance

All activities must adhere to licensing and regulatory standards, as well as company policies.

Work Environment
