Position Summary:

As a Recovery Assistance Specialist (RAS), you will play a crucial role in guiding clients as they establish and pursue their recovery objectives. Utilizing the ACT Model, you will work within a collaborative, multi-disciplinary team to deliver intensive rehabilitative services, support, and interventions that empower clients to reach their personal recovery milestones. This position involves equipping clients with essential skills to manage their conditions effectively and promote independent living.

Key Responsibilities:
  • Conduct ongoing evaluations of clients' needs and facilitate the creation and revision of Individualized Recovery Plans.
  • Educate clients on effective illness management through skill development in symptom recognition, coping strategies, and daily living.
  • Support clients in acquiring skills that promote independence and help achieve recovery goals, including interpersonal and transportation skills.
  • Provide assistance during crisis situations to ensure client safety and well-being.
  • Guide clients in applying for necessary benefits and entitlements, such as SSI and Medicaid.
  • Help clients build natural support networks by connecting them with community resources like self-help groups and recreational facilities.
  • Assist clients in finding safe and affordable housing that meets their care requirements.
  • Accompany clients to essential services, particularly medical appointments.
  • Offer education and support to clients' families and support systems as needed.
  • Document all services rendered promptly, including billing and progress notes, and maintain accurate records of communication.
  • Monitor clients' engagement with external services.
  • Participate in daily team meetings to coordinate care.
  • Be part of the on-call system to provide after-hours support.
  • Advocate for clients as they navigate social and community systems.
  • Attend all mandatory training sessions.
  • Perform additional duties as assigned.
Qualifications:
  • A Bachelor's or Master's degree in a human services field is preferred, or a high school diploma with relevant experience.
  • Strong written and verbal communication skills.
  • Proficient in basic computer applications, including Microsoft Office.
  • Able to thrive in a diverse and dynamic work environment.
