Job Summary

We are seeking a highly motivated and compassionate Service Coordinator to join our team at The Salvation Army USA Western Territory. As a Service Coordinator, you will play a vital role in identifying the human service needs of our residents and developing service plans to support their independence and well-being.

Key Responsibilities
  • Conduct daily assessments to identify the needs of residents and develop individualized service plans.
  • Collaborate with local service agencies and resources to provide on-site services and referrals.
  • Educate residents and staff on service provision and aging in place issues.
  • Assist the Manager in promoting resident independence and addressing related issues.
Requirements
  • Experience working with the elderly and knowledge of aging issues, including disability, mental health, and abuse.
  • Knowledge of Fair Housing laws and regulations, HUD program requirements, and local service agencies.
  • Strong communication and interpersonal skills.
  • Bachelor's Degree in Gerontology, Social Work, Psychology, or related field, or equivalent experience.
