Discharge Planning Coordinator

We are seeking a skilled Discharge Planning Coordinator to join our team at MLK Community Healthcare. As a key member of our multidisciplinary team, you will be responsible for coordinating the discharge/transition process for patients, ensuring a seamless transition from hospital to post-acute care.
Key Responsibilities:- Coordinate continuity of patient care with external healthcare organizations and facilities.
- Assist patients through the healthcare system, under the direction of CM, by operating as a patient advocate and health systems navigator.
- Obtain patient choice for post-acute facilities as required by CMS Conditions of Participation.
- Report care/discharge barriers to appropriate care manager.
- Two (2) years continuous recent experience in a healthcare setting as unit clerk /care coordinator or similar position required.
- A team player that can multitask and can follow details – knowledge of CMS guidelines preferred.
- Highly organized and well-developed oral and written communication, problem-solving, and decision-making skills.
- An estimated salary range of $60,000 - $80,000 per year, depending on experience.
