Key Responsibilities:

  • Developing and implementing case management plans to optimize patient outcomes and satisfaction
  • Collaborating with interdisciplinary teams to address patient needs and identify opportunities for growth and improvement
  • Participating in quality improvement initiatives to enhance the discharge planning process

Requirements:

  • Minimum 3+ years of experience in pediatric nursing, with a focus on utilization management and discharge planning
  • Strong communication and negotiation skills to effectively engage with patients, families, and healthcare professionals
  • Ability to navigate complex healthcare systems and coordinate care across multiple settings
  • Familiarity with managed care principles and continuous quality improvement methodologies

Salary Range:

The estimated salary for this role is $110,656.00-$190,882.00 per year, depending on qualifications and experience.
