About the Role:

Martin Luther King, Jr. Community Hospital seeks a skilled Nurse Care Coordinator to join its emergency department team. As a key member of our interdisciplinary team, you will play a crucial role in coordinating patient care from admission through discharge.

Job Summary:

The Care Manager is responsible for managing the continuum of care for assigned patients, ensuring high-quality outcomes and efficient utilization of healthcare resources. This includes partnering with medical staff, utilizing scientific evidence, and relevant data to determine medical necessity, quality of care, and resource allocation.

Responsibilities:

  • Collaborate with the interdisciplinary team to facilitate timely care, ensure quality of care throughout the hospital stay, and minimize adverse outcomes.
  • Initiate appropriate referrals to the internal interdisciplinary team.
  • Communicate with Admitting or PFS regarding patient needs, payer, and provider support within existing benefit structures.
  • Establish a working diagnosis (DRG) on every patient at admission to estimate target length of stay or identify date of discharge for planning and care coordination purposes.
  • Communicate target LOS/estimated discharge readiness to physician, patient, family, care team, and payor.
  • Document all team, physician, and patient/family communication and concerns related to care coordination and services.
  • Screen every patient to identify need for further assessment of medical necessity or discharge planning.

Requirements:

  • Bachelor's degree in nursing preferred; Associate's degree in nursing required.
  • Minimum one to three years of hospital or related experience required; internals with 18 months acute care case management/coordination experience may be considered in lieu of nursing clinical experience.
  • Able to navigate and connect successfully with outside provider networks (Health Plans, IPA's, and FQHC's).

Preferred Qualifications:

  • Bilingual language skills (Spanish).
  • Current California Nursing license.
  • Basic Life Support (BLS) certification.
  • Case Management certification.

Compensation:

We offer a competitive salary of approximately $85,000 per year, based on experience, plus benefits and opportunities for growth and development.
