About the Role:
The University of Vermont Health Network is seeking a skilled LPN to join our team as a Clinical Resource Coordinator. This role will work closely with our Population Health Services Organization to support High Value Care performance initiatives and clinical workflows.
Key Responsibilities:
- Work under the direction of the AVP Population Health Services to support data-driven initiatives and clinical workflows.
- Maintain a working knowledge of High Value Care and PHSO strategy.
- Collaborate with Population Health Services Organization and Medical Group teams to identify patients' unmet needs and engage care teams in care.
- Enhance ongoing communication and coordination with care teams.
Requirements:
- Graduate of an accredited Licensed Practical Nursing Program.
- One to two years of experience in a relevant healthcare setting, with primary care experience preferred.
