Director of Care Coordination and Patient Experience Leader

We are seeking a highly skilled Director of Care Coordination and Patient Experience to join our team at LifeCare Home Health. This role is responsible for building and managing our client care coordination department to deliver a seamless experience for our patients, removing barriers to treatment, and building collaborative relationships with patients, referral sources, and other providers.
Key Responsibilities:
- Manage the daily activities of the Care Coordination Service Center.
- Identify and track relevant data to analyze the success of the Care Coordination team, evaluating individual performance and coaching team members on areas of improvement.
- Establish and operate a structured approach to quality using industry-relevant service metrics.
- Coordinate the design and implementation of new tools, programs, processes, and procedures to build out the care coordination function at LifeCare.
- Resolve escalated client issues that are beyond team member ability.
Requirements:
- RN or LPN licensure.
- At least 2 years of experience managing a service center for healthcare coordination, case management, or care management.
- At least 3 years of experience managing and developing multiple direct reports.
- At least 3 years of experience with data analysis to improve service performance.
- Competency in office productivity and collaboration tools.
- Patient/client experience preferred.
- Centralized intake experience preferred.
- Centralized on-call experience preferred.
- Call-back program experience preferred.

Licensure/Certification:
- Current driver's license in good standing.
- Reliable transportation and auto liability insurance.
- Current and unencumbered professional license as an RN in any/all applicable states.
Physical Requirements:
- Prolonged sitting required in an office environment.
- Ability to handle stressful situations in a calm and courteous manner at all times.
- Requires working under some stressful conditions to meet deadlines and Agency needs.
Environmental/Working Conditions:
- Work is performed in a home office environment.
- Work requires considerable sitting and close-up vision work.
- Continually communicates with staff and other health care professionals utilizing email and telephone systems.
- Must be able to articulate in a professional manner.
- Typical weight lifted is five pounds; maximum weight lifted is ten pounds.
