Responsibilities

  1. Assess, plan, implement, and evaluate patient care plans to ensure optimal outcomes.
  2. Collaborate with multidisciplinary teams to coordinate patient care and services.
  3. Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions.
  4. Maintain accurate and detailed records of patient interactions, treatments, and progress.

Requirements

  • Baccalaureate degree in nursing (BSN) from an accredited institution.
  • Licensure as a registered nurse in New York State.
  • Demonstrated ability to work collaboratively in a fast-paced environment.
  • Strong communication and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive information.

Salary and Benefits

We offer a competitive salary range of $32.00 - $38.60 per hour, based on experience and qualifications. Our comprehensive benefits package includes health, dental, and vision insurance, retirement savings plan, and paid time off.
