Job Description:
We are seeking a dedicated Registered Nurse Care Coordinator to join our team. This full-time role will focus on providing essential care coordination for complex patients in collaboration with the VaH provider team.
Responsibilities:
- In-home patient assessments
- Patient education and care plan development
- Covering the nurse triage line and lab specimen collection
- Vaccine administration when needed
Requirements:
- Bachelor of Science in Nursing from an accredited nursing program
- 1 year of registered nursing experience required, 3 years strongly preferred
- Specialty Board Certification strongly preferred
- Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia
Compensation:
$85,820.80 - $113,401.60 Annual
