In this dynamic and rewarding position, you will be responsible for delivering comprehensive patient care through the implementation of evidence-based practices. You will work collaboratively with interdisciplinary teams to ensure seamless patient transitions, optimize health outcomes, and promote positive patient experiences.

Key Responsibilities:

  • Coordinate patient care services, including developing and implementing individualized care plans.
  • Collaborate with healthcare providers to ensure continuity of care and timely interventions.
  • Provide education and support to patients and families to enhance their understanding of treatment options and self-care strategies.

Requirements:

To be considered for this role, you must have a graduate degree in nursing and at least one year of relevant experience. A valid license to practice as a Registered Nurse in the Commonwealth of Virginia is also required. Additionally, American Heart Association (AHA) Health Care Provider BLS certification is necessary for this position.

Work Environment:

This role requires standing for prolonged periods, frequent travel, and bending/stooping. You will need to be proficient in communicative, auditory, and visual skills, with attention to detail and ability to write legibly. Lifting/pushing/pulling 20-50lbs may be required, and you may be exposed to chemicals, blood/body fluids, and infectious diseases.
