We are seeking an experienced and dedicated Nurse Care Coordinator to join our team at the University of Virginia. As a key member of our healthcare team, you will play a critical role in ensuring seamless transitions for our patients.
Job SummaryThe Registered Nurse Care Coordinator is responsible for coordinating care activities between multiple participants, using advanced critical thinking to develop, implement, and evaluate individualized care plans. You will work closely with patients, families, and interdisciplinary teams to ensure effective communication, coordination, and follow-up.
Duties and Responsibilities- Develop, implement, and evaluate individualized care plans that empower patients and optimize outcomes.
- Maintain safety and continuity of care through documentation, hand-off tools, and other methods.
- Collaborate and advise patients, families, and caregivers in their healthcare decisions, respecting their culture and values.
- Provide health education and coaching tailored to identified treatment and service plans, adhering to evidence-based care delivery and safety standards.
- Participate in and apply research and evidence-based practices to improve patient care throughout the lifespan and across the continuum.
- Take the lead in ensuring continuity and consistency of care across the continuum, promoting pre-visit coordination, post-clinic follow-up, and handoff between services.
- Educate patients and families about evidence-based standards of practice to empower self-management strategies.
- Identify support needs, develop action plans, and guide patient-centered care planning and application of the nursing process.
- Contribute to problem-solving through communication and collaboration, evaluating outcomes of treatment options and tracking patient progress toward care plans and goals.
- Bachelor of Science in Nursing from an accredited nursing program.
- 1 year of registered nursing experience required, 3 years of related and relevant registered nursing experience strongly preferred.
- Specialty Board Certification strongly preferred.
- Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification is required.
This position requires standing for prolonged periods, frequent travel, and bending/stooping. Proficient communicative, auditory, and visual skills; attention to detail and ability to write legibly; ability to lift/push/pull 20-50lbs. May be exposed to chemicals, blood/body fluids, and infectious diseases.
Compensation and BenefitsThis position offers a competitive salary range of $85,820.80 - $113,401.60 annually, commensurate with experience.
