The compensation for this role ranges from $24 to $32 per hour, with variations based on factors such as geographic location, experience, and education. Successful candidates will have the opportunity to transition to permanent positions, gaining access to comprehensive benefits, paid time off, and additional perks.

Key Responsibilities:

  • Support residents in accessing and maintaining necessary medical and mental health services.
  • Assist residents in navigating entitlement programs.
  • Facilitate home care arrangements for residents.
  • Coordinate care among various community service providers as needed.
  • Help residents foster and maintain family and social connections.
  • Encourage resident participation in social activities through referrals.
  • Guide residents in budgeting and income management to ensure timely rent payments.
  • Provide assistance with financial management as required.
  • Follow up with residents and service providers during off-site stays, such as hospitalizations.
  • Oversee discharge planning processes, including necessary documentation.
  • Offer individual counseling sessions as needed.
  • Deliver crisis intervention services when required.
  • Conduct group work services for residents, focusing on recreational, educational, and clinical activities.
  • Communicate resident progress and needs with the team as appropriate.

Qualifications:

  • A Bachelor's Degree in Social Work or Human Services is required; a Master's Degree is preferred.
  • Bilingual proficiency in Spanish and English, both written and verbal, is essential.
  • Experience working with older adults and individuals facing mental health or substance use challenges is preferred.
