Job SummaryThe Licensed Practical Nurse serves as a vital member of the healthcare team at UVA Health Northern VA & Culpeper, utilizing best scientific evidence and compassion to assist patients during their health journey. You will accept accountability for provision of care, as part of the RN-led team, in accordance with current policies and procedures.
Responsibilities- Provides evaluation and management of patients
- Participates in the collection of patient data as needed
- Determines nursing care needs through recognition of relationships between data gathered and a patient's health status and treatment regimen
- Identifies relevant resources based on necessity and availability
- Communicates information to others responsible for or involved in the care of the patient
- Participates in dialogue with the patient/chosen support system to identify the individualized care plan
- Consistently utilizes appropriate communication tools such as Situation, Background, Assessment, Recommendation, and Question (SBARQ) when communicating within the interdisciplinary team and when handing off patient care
- Documents appropriate information in a timely manner
- High School Diploma required
- Graduate of accredited Practical Nursing Program required
- Completion of a nursing program as approved by the State Board of Nursing
- 1 year of relevant experience preferred
- Applicable state LPN license required
- Ability to multitask and prioritize quickly
- Ability to communicate verbally and in written form
- Basic computer knowledge
The job requires constant standing, frequently walking, and bending/stooping. Proficient communicative, auditory, and visual skills are necessary. Attention to detail and ability to write legibly are also required. Ability to lift/push/pull 100 lbs. For any weight over 35 lbs., use Safe Handling Equipment.
OtherMay require the use of safety equipment, such as HEPA mask, for infection prevention. On call responsibilities as directed. Ability to travel between campus buildings, remote facilities, and out of town as needed.
