Job Title: Resident Care Coordinator

TerraBella Salisbury is seeking a highly skilled and compassionate Resident Care Coordinator to join our team. As a key member of our healthcare team, you will be responsible for coordinating resident care needs, ensuring the highest level of quality care and service.

Responsibilities:
  • Coordinate resident care needs, including scheduling, documentation, and communication with healthcare providers.
  • Assist the Director of Health & Wellness in training new care managers and medication technicians.
  • Supervise and deliver resident care services in compliance with TerraBella expectations, policy, and procedure, and state regulations.
  • Ensure resident care staffing is within regulatory and company requirements.
  • Perform daily filing of resident records, quality assurance reviews, and cart audits.
  • Assist with data collection, reporting, and medication administration processes.
  • Provide preceptorship to new team members, ensuring compliance with policies and procedures.
  • Notify the Director of Health & Wellness of changes in resident condition and unusual observations.
  • Schedule appointments and arrange transportation for residents.
  • Implement fire safety and disaster plans, and evacuation procedures.
Qualifications:
  • Meet minimum criteria to perform as a Supervisor in Charge in the State of North Carolina.
  • Be a licensed Medication Technician, CNA with at least one year of experience preferred.
  • Possess excellent written and verbal communication skills.
  • Express interest in working with a senior population.
  • Demonstrate a warm, outgoing, and compassionate personality.
  • Have knowledge of community agencies and appreciation for social and life history status of residents.
