Position Overview for Care Coordination Specialist
The Care Coordination Specialist (CCS) at the Peninsula Regional Clinically Integrated Network (PRCIN) plays a vital role within the primary care practice team. Under the guidance of the PRCIN Care Coordination Supervisor, the CCS is tasked with delivering care coordination services to patients identified as moderate to high risk, ensuring safe and effective transitions throughout the healthcare continuum, which includes outpatient care, hospitalization, home settings, and skilled nursing facilities. The CCS acts as a resource and coach, emphasizing health and wellness, preventive care, and the social determinants of health. In collaboration with the healthcare team, the CCS develops and implements a patient-centered care plan that encourages self-management.
Key Responsibilities
- Conduct practice-based care coordination for patients at risk of health decline, adverse outcomes, or high resource utilization.
- Identify social, environmental, and financial needs, coordinating referrals to appropriate community resources.
- Encourage patients to complete specific healthcare tasks aimed at improving their health and provide disease-specific education to support self-management.
- Collect and analyze data related to team and population health metrics.
Qualifications for Care Coordination Specialist
Education:
- Bachelor's degree in Community Health Education or Health Sciences, or a minimum of 4 years of experience in a clinical or administrative role within an acute care or community outpatient environment.
- Familiarity with EMRs - Epic preferred.
- Proficient in Microsoft Word, Excel, and PowerPoint; experience in data management, analysis, and entry.
- Experience in community health, emergency departments, outpatient medical offices, or inpatient hospital settings preferred.
- Valid driver's license and ability to travel to meet with patients and providers.
- Preferred experience in patient education and engagement.
- Knowledge of local community resources.
Work Schedule
- Monday to Friday, day shift. No holidays.
- May require flexibility to meet the needs of patients and providers outside of standard hours.
Benefits
- At TidalHealth, full-time employees working at least 72 hours per pay period, part-time employees working at least 36 hours per pay period, and part-time employees working 30 hours or more on weekends are eligible for benefits.
