Community Care Coordinator

We are seeking a skilled Primary Clinician to join our team at Riverside Community Care.
The ideal candidate will have a Master's degree in Social Work or Mental Health Counseling, licensure within 2 years of hire, and at least one year of related experience.
About the PositionAs a Primary Clinician, you will:
- Provide care coordination, person-centered assessment, and service planning to support individuals in our ACCS program
- Establish therapeutic relationships with individuals and their families
- Develop and implement person-centered plans to support independence and community tenure
- Coordinate care with other providers and community supports
