We are seeking a highly skilled ESRD Care Coordinator to join our team at American Renal Associates. As a Licensed Clinical Social Worker, you will play a critical role in helping patients understand their rights and responsibilities, guiding them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.

Responsibilities
  • Assess patient needs and develop personalized care plans to ensure optimal treatment outcomes
  • Educate patients on disease management, treatment options, and available resources
  • Collaborate with healthcare teams to coordinate care and address patient concerns

To be successful in this role, you will require:

Requirements
  • A Master's degree from an accredited school of social work (MSW)
  • Current state licensure (e.g. LCSW, LMSW), meeting all state requirements for practice in a dialysis setting
  • Two years of clinical work experience, preferably in an ESRD setting
  • Excellent communication and interpersonal skills

We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience, plus benefits that include annual raises significantly above the industry standard and paid time off accrual based on your most recent dialysis experience.
